AUTHORIZATION FOR THE TRANSFER OF HUMAN REMAINS

To facilitate the transfer of human remains to the Republic of Congo, please ensure that the following documentation is provided:

1. Birth Certificate: A copy of the deceased's original birth certificate.
2. Proof of Citizenship: Proof of Congolese citizenship, such as a Congolese passport, Congolese National ID card, or Congolese citizenship certificate.
3. Medical Records: A copy of the hospital’s medical records certifying the death and its causes for the deceased Congolese citizen.
4. Funeral Coroner’s Office Confirmation: An official letter on the Funeral Coroner’s Office letterhead confirming that

1. Birth Certificate: A copy of the deceased's original birth certificate.
2. Proof of Citizenship: Proof of Congolese citizenship, such as a Congolese passport, Congolese National ID card, or Congolese citizenship certificate.
3. Medical Records: A copy of the hospital’s medical records certifying the death and its causes for the deceased Congolese citizen.
4. Funeral Coroner’s Office Confirmation: An official letter on the Funeral Coroner’s Office letterhead confirming that they are responsible for making the necessary arrangements to ship the human remains to the Republic of Congo.
5. Embalming Certificate: A certificate from the Funeral Coroner’s Office, on official letterhead, attesting that the human corpse was embalmed according to proper and legal British. sanitary regulations and does not pose any health threat or disease to anyone.
6. A copy of the Funeral Coroner’s Office license that oversees shipping the human remains to the Republic of Congo.
7. Family Representative Letter: A signed and typed letter by the family representative residing in either the United Kingdom of Great Britain and Northern Ireland or the Republic of Ireland, addressed to the Embassy of the Republic of Congo in the UK, requesting a death certificate. The letter should include your full name, home address, email address, date, and telephone number.
8. Family Representative’s ID: A copy of the family representative’s identification card (Driver’s License, Passport, or Consular Card).
9. Flight Itinerary: A flight reservation or flight itinerary confirming the intention to transfer the human remains to the Republic of Congo.
10. Payment: The fee is £40, payable only in cash if you are physically present. Please note that all consular service fees are non-refundable.
11. Prepaid Return Envelope: A prepaid, trackable special delivery envelope should be included for the return of your authorization for the human remains transfer.
